A Balanced Approach to Comfort and Care

It’s not unusual for children to feel anxious about dental visits. As parents and caregivers, we want them to feel safe and supported. Fortunately, modern pediatric dentistry uses a wide range of tools—from gentle communication to safe sedation techniques—to help make dental care a positive experience.

Start with Trust: Nonpharmacological Behavior Guidance

Many children can be calmed using evidence-based, nonpharmacological behavior guidance. These techniques focus on communication, modeling, distraction, and environmental adjustments to ease anxiety and improve cooperation.

Some common strategies we used in our clinic include:

Tell-Show-Do (TSD): Explaining and demonstrating procedures using child-friendly language and visuals.

Modeling: Letting children observe a sibling or parents (or even their stuffy) calmly undergoing care.

Audiovisual distraction: Using tablets or videos to shift focus away from dental procedures.

Positive reinforcement: Praising cooperative behavior to encourage calmness and confidence.

Cognitive behavior techniques: Including relaxation breathing and imagery for older or more cognitively mature children.

Sensory-adapted dental environments (SADE): For children with special healthcare needs, adapting lighting, sound, and sensory inputs can dramatically reduce anxiety.

These approaches are often enough to support preventive care and minor treatments, and research shows they can significantly reduce anxiety, particularly when customized to a child’s personality and developmental level

When Sedation May Be the Kinder Option

Despite the best efforts, some children—especially toddlers, children with special needs, or those needing extensive or invasive treatment—may find even basic care overwhelming. This is where sedation becomes an important and safe adjunct.

Sedation is not about “knocking kids out”; it’s about helping them cope with care they’re not yet able to handle on their own. Some common situations where sedation is helpful include:

A very young age, when children cannot understand instructions or sit still.

An extensive treatment plan that would be too much to tolerate over multiple visits.

Procedures that are uncomfortable or complex, like extractions or nerve treatments.

Sedation exists on a spectrum and we will choose the least invasive approach appropriate for your child.

Types of Sedation we provide in our office:

Nitrous Oxide (“Laughing Gas”)

A mild, inhaled sedative that relaxes your child while keeping them fully awake. It’s fast-acting, reversible, and widely used for its safety and comfort.

Moderate Sedation (Oral Medication + Nitrous Oxide)

Combines laughing gas with prescribed medications. Your child becomes drowsy but remains responsive. They may or may not remember the procedure, which helps prevent long-term fear.

Deep Sedation / General Anesthesia

Reserved for children with complex medical needs, severe anxiety, or extensive treatment plans. This is done in a hospital setting where your child is completely asleep and unaware of the procedure.

All sedation is conducted under strict monitoring protocols according to AAPD guidelines.  We adhere strictly to all safety measures include continuous monitoring of breathing, heart rate, and oxygen and carbon dioxide levels throughout and after the procedure.

My daughter had a wonderful experience with Dr. Phoebe and her dedicated team. She is meticulous, careful, and wholeheartedly committed. She explained all the necessary treatments for my daughter and provided the most helpful advice tailored to her age and specific condition, ensuring my daughter felt no pressure during her dental care. Highly recommend Dr Tsang and her team. Dental procedures can be very scary, especially for your little one, and biggest fear is if the dental team is over charging or over prescribing. I can honestly say you can trust Dr Tsang and her team. I was impressed by the patience and gentle approach she used when explaining all our options, answering my list: if questions. She did not automatically overprescribe yet she understands what’s needed to ensure my 5 year old daughter left with the best experience. My daughter used oral sedation with Midazolam (Short acting Benzo that is easily reversible and safe) which is much less invasive than general anesthetic route often recommended by other pediatric dentists first As well Dr Tsang was honest when updated X-rays were taken that there were fewer than needed cavities to be filled than we had expected Not to mention they had so many goodies and prizes for my daughter throughout the various steps of process, that she left as if she just went to a birthday party. To summarize, if you’re looking for a very knowledgeable but humble professional, whom you can trust to not over prescribe yet is willing to ensure your child has the most comfortable and positive dentistry experience, Dr Tsang and her team embodies all those traits above.

Our 2.5-year-old son, Howard, recently started experiencing pain in his upper right back teeth. Concerned, we took him to a nearby general dentist, who, after a thorough examination, referred us to a pediatric dental specialist, Dr. Phoebe Tsang. To our surprise, Howard had extensive dental issues, requiring treatment for 14 teeth through surgery. As parents, we felt deeply sorry that he had been enduring more discomfort than we had realized, especially since he was too young to fully express his pain. Howard had been drinking milk at night since birth, and we were unaware of the significant impact it could have on his teeth. However, after our initial visit to the general dentist, he successfully transitioned away from night bottles. Determined to provide him with the best care, we made the one-hour drive from White Rock to see Dr. Tsang. From the moment we arrived, Dr. Tsang and her team were incredibly professional, compassionate, and gentle with Howard. Dr. Tsang herself is not only highly knowledgeable but also exceptionally skilled in pediatric dentistry. We feel truly fortunate to have had her perform Howard’s surgery. Yesterday, on March 18, 2025, Dr. Tsang and anesthesiologist Dr. Robert carried out Howard’s procedure with exceptional expertise and care. We are incredibly grateful for their dedication. Post-surgery, Howard has been recovering well—he is experiencing minimal discomfort and has been able to drink formula and eat soft foods comfortably. This was Howard’s first surgery, and under Dr. Tsang’s guidance, we are confident that he will continue to improve and eat with ease. We sincerely appreciate the outstanding care provided by Dr. Tsang and her team and wholeheartedly recommend her to any parents seeking a skilled pediatric dentist for their child’s dental needs. Highly recommended Dr Phoebe Tsang! Our 5 year old son was very anxious at our local dentist and we did not want him going under anesthetic for his teeth. Thankfully we got a referral to Pediatric Dental Specialist Dr Tsang, who uses conscious sedation. From the first FaceTime consultation, she was so kind! Our son was at ease and really looked forward to meeting her. She thoroughly explained the procedure and answered all our questions with patience. She is unbelievably knowledgeable and gave us reassurance that everything would go well. Upon arrival at her beautiful office, we were greeted with the utmost love from not only her but her whole team. Being in her care was top of the line, we cannot thank her enough for making this go so smooth for our family. If your children need dental work done, look no further. I guarantee you, she is the absolute best you will find!

My 4 year old son has many sensory issues and brushing his teeth was absolutely impossible, which is what brought us to Dr Tsang. He needed a ton of work done for his mouth to be healthy again. I was blown away by her and her staff from start to finish. She treated my son so well and I was so impressed with her efforts to connect with him in a way that would work for him. He did need dental surgery, which scared me but all the questions I had were answered so quickly and there was a great deal of communication from his first appointment, all the way up to the surgery. You can tell how much Dr Tsang and her staff care about their patients. I couldn't be happier with my son's care. I truly recommend Dr Tsang and staff if your child needs dental work done. They have so much compassion and throughout the entire process I felt like my son was in great hands. I'm so thankful to everyone who worked with him and those I had contact with over many months. This was a very stressful situation for me and it was made so much better by everyone there.
